The GPs at Tallaght Cross are committed to a vision of General Practice we call “Tomorrow’s Care”. We are seeking to introduce new ways of improving health and well-being for our patients, with initiatives including Social Prescribing, targeting a reduction in unplanned hospital admissions and also we will shortly have a Pharmacist as part of our Practice Team.

Social prescribing refers to the use of non-medical interventions to help patients. We have linked in with almost 300 community supports in the Tallaght area, covering a wide range of services and supports. When we identify that a you may benefit from being linked in with one of these organisation we will ask our Social Care Coordinator, Brenda Nolan to contact you and make a personalised plan, putting you in touch with the supports that you need.

With more and more medications being prescribed for our patients it can get very challenging to manage medications efficiently. Many of our patients are on multiple repeated medications. We want to bring a pharmacist into our practice team, this has worked really well in other countries and often results in reduction of total number of medications, cost-savings and smoother communications when new medicines are added, for example in the hospital setting. The pharmacist will be available to directly answer your questions in relation to your tablets, side-effects, duration etc.

We are committed to keeping you well, in the community. We know how upsetting it can be being admitted to hospital, particularly when it was not expected. We will endeavour to maximise your health and links with all relevant health professionals to maintain you healthy, at home!
